Long-form Birth Certificate

A long-form birth certificate is a certified copy of a birth registration that can be used for official purposes. Long-form birth certificates must be issued by the Vital Records Office in the state where the birth occurred and was registered.

As opposed to the short-form, the long-form certificate is an exact copy of and contains the same information as the original birth certificate, which is always kept on file at the office where it was created.
